        ESWL में शरीर के बाहर से शॉकवेव भेजकर पथरी को छोटे टुकड़ों में तोड़ा जाता है, बिना किसी चीरे या सर्जरी के। यह छोटी और नरम पथरी के लिए अच्छा रहता है। बड़ी या सख्त पथरी में लेज़र तकनीक ज़्यादा असरदार होती है, क्योंकि इसमें सीधे पथरी तक पहुंचकर उसे तोड़ा जाता है। सही तरीका पथरी के साइज़, जगह और सख्ती को देखकर तय होता है।

        लेज़र सर्जरी में छोटे-छोटे कट या बिना चीरे के इलाज होता है, जिससे दर्द कम, खून कम बहता है और रिकवरी तेज़ होती है। ओपन सर्जरी में बड़ा चीरा लगता है और ठीक होने में ज्यादा समय लगता है। हर मामले में लेज़र सही नहीं होता — पथरी के साइज़, जगह और मरीज की सेहत देखकर डॉक्टर तय करते हैं कि कौन सा तरीका सही रहेगा।
